Ontario Patient Referrals
All patients with moderate-to-severe hip osteoarthritis will be assessed through Central LHIN’s Musculoskeletal (MSK) Rapid Access Clinic (RAC). The RAC offers patients an expedited and comprehensive assessment with an Advanced Practice Physiotherapist (APP).
If the patient is a surgical candidate, they will be scheduled for an appointment with Dr. Rodriguez-Elizalde on a different day. The RAC provides patients with timely access to care and helps reduce wait time (Wait 1).
For general orthopaedic referrals, such as trauma surgery, fracture care, or injections, please fill out and fax the referral form provided along with any relevant diagnostic imaging reports to 416-636-8628.
Please include the patient’s past medical and surgical history, current medications, specialist notes and recent blood work (if relevant).
All referrals will be triaged upon receipt and processed within 14 business days. We will contact patients directly with an appointment.
Patients are expected to bring a copy of their x-rays, MRIs, and/or CT images on disc or film on the day of their appointment. Reports alone are typically insufficient.
Patients who have had their studies ordered at Humber River Hospital need not arrange for pick up. We will access the films for them. Patients must bring a valid health card, and shorts (hip/knee issues).
Please note, Dr. Rodriguez-Elizalde does not specialize in spine, foot, ankle, wrist, or shoulder and elbow issues. For referrals such as these, please refer to appropriate specialty.
Out of Province Patient Referrals
Dr. Rodriguez-Elizalde offers surgical consultations and solutions for out of province patients at the Clearpoint Surcial facility in Toronto. For inquires about expedited care, please contact Mark Angelo directly at: mark.angelo@clearpointhealth.ca

International Patients Seeking Surgical Consultation and Surgery
Does Dr. Rodriguez-Elizalde operate on International patients?
Yes. Non-OHIP, or out-of-country patients who want surgery are candidates based on the same criteria Dr. Rodriguez-Elizalde uses to screen regular elective patients. Most International patients require a consultation, x-rays, and a review of their clinical condition in person prior to
surgery.
The length of stay for each patient varies, depending on the procedure, family availability, insurance coverage, and travel plans. The hospital has set rates depending on the procedure, and often these accounts must be settled prior to surgery happening.
If you would like more information, or to schedule a consultation, please fill out the referral form and contact our office. Referral by another physician is not necessary in the case of International patients.
Sebastian is an Orthopedic Trauma and Adult Hip and Knee surgeon at Humber Hospital, in Toronto, with a special interest in revision and complex cases.
He completed his orthopedic surgery residency at the University of Ottawa, after which he was accepted for Fellowship training at the Hospital for Special Surgery in New York, in Hip and Knee Arthroplasty.
